I hereby give permission to Reap and Prosper Sports transport my child to/from athletic events involving Reap and Prosper Sports games and or trainings for the 2023-2024 season.

I understand that by signing below I am giving the above-mentioned permission to transport my minor child in a private vehicle not insured by or through Reap and Prosper Sports. I understand that this transportation agreement releases Reap and Prosper Sports and it’s affiliates from responsibility during the transportation. I understand that although the volunteer is associated with and has a current background check through the league and training, Reap and Prosper Sports holds no liability or responsibility for personal injury, damages, loss of persons or property that may occur during the transportation.
